Steps to Change the Merchant Bank Account

1. Please request the form to change your account from your single point-of-contact service manager.

2. Please sign and complete this form, and send back with a copy of an imprinted, voided check. If this is impossible to obtain, please provide a bank letter on bank letterhead that lists the account name, routing number, and account number.
